Waiver and Release agreement
All persons participating in tennis, racquet sports, wellness activities, lessons, clinics, leagues, tournaments or other recreational activities at Luna Racquet & Wellness Club must have a current waiver on file before participating.
Adult Participant (18 and over)- Complete Waiver below.
Minor Participant (18 and Under) - Parent/Natural Guardian must complete Minor Waiver
